COVID-19 Test Results Request

  1. Please note, we are unable to provide test results if you were tested by a medical provider; please contact that provider directly for your results.

  2. There are delays in test results that we cannot control. If you submit this request, someone from the public health department will call you within 48 hours to follow up either way. The National Guard makes 2 attempts to contact patients with negative test results. Walworth County Public Health will follow up with positive test results as they are processed. Please only use this form if it has been more than 7 days since you were tested and you do not have your results. Please provide:
