Take the Walworth County Community Health Survey
The Walworth County Department of Health & Human Services is conducting a survey to hear more about what it is like to live, learn, work, worship, and play in Walworth County.
The survey is part of Walworth County’s Community Health Improvement process. Every three to five years, we take a step back and ask our community what the most important health issues and needs are for Walworth County. Participating in the Community Health Survey allows your voice and the voice of your neighbors, friends, and family to be heard. Survey results will be used to identify needs, set goals, and work with community partners to improve the health and well-being of all who call Walworth County home.
How to Take the Survey
Complete the Community Health Survey online using this link or print the survey (English version, Spanish version) and mail it to:
