CCS Plan
The CCS Plan is a written document, required under Chapter DHS 36 of the Wisconsin Administrative Code that describes the plan to implement the Comprehensive Community Services (CCS) benefit in Walworth County. This covers such items as the organizational structure, policies and procedures, the array of psychosocial rehabilitation services and service providers to be available through the CCS program.

Consumer Satisfaction Surveys
Consumer satisfaction surveys will be sent out annually to all CCS consumers. Completion of the surveys is voluntary, but is highly recommended in order to make your opinion count. The results are confidential. The tools used include:
- Mental Health System Improvement Project (MHSIP) Adult and Youth Satisfaction Surveys – These surveys ask about experiences and interactions with staff in the past (6) months.
- Mental Health System Improvement Project (MHSIP) Family Satisfaction Survey – is a survey completed by the parent or guardian or children age 12 and under. It asks about satisfaction with the mental health and/or substance use services their child has received in the past (6) months.
