Lakeland Health Care Center’s (LHCC) Social Services Department is responsible for independently identifying and documenting the psychosocial and emotional needs of residents.
Services Include:
- Address complaints by residents or family members.
- Addressing the psycho social needs of residents and their family members, and offer support.
- Assist in discharge planning, including coordinating with community resources regarding alternate placement from LHCC to other community/health care facilities. Arrange resident/family care conferences.
- Assist residents in maintaining their highest level of functioning and positive existence.
- Collaborate with family and physicians to make hospice referrals when appropriate, and address end of life issues.
- Partner with psychiatric providers to ensure that resident’s mental health needs are met.
- Provide educational events to the residents, staff, and the community.
- Provides information to residents/family regarding advance directives.
