Vital Records (Births, Deaths & Marriages)

Effective January 3, 2017 State of Wisconsin has statewide issuance of vital records for births from October 1, 1907 to present, deaths from September 1, 2013.  Effective January 2, 2020 State of Wisconsin has statewide issuance of marriages.  Death certificates prior to September 1, 2013 must be obtained at the occurrence or residence county only.  Applicants must have a direct and tangible interest in order to purchase a certified copy of any vital record. 

In Person
Applications submitted in person in our office generally take 15 to 20 minutes to complete. The cut off time for processing in-person requests is 4:30 p.m. Note: at least one form of valid identification which contains your current name is required to complete each request.    

By Mail
Complete the appropriate form(s) below, include a copy of your form of identification, a self addressed stamped business sized envelope and applicable fee. In most instances, mail orders received are processed on the same day the day following.

Online Orders

In most instances, online orders are processed on the day received or the day following. For online orders, please change the shipping method to U.S. (Standard) Mail if you do not wish to use the express mail service.
