to Lakeland Health Care Center
Lakeland Heath Care Center is owned
and operated by Walworth County. Lakeland Heath Care Center is
licensed to provide care under Medicare and Medicaid (Medical Assistance). Long
term care insurance, private funds and a variety of insurances are accepted.
An interdisciplinary team approach to
care helps residents to attain and maintain their highest level of
independence.
Lakeland Heath Care Center provides a wide variety of
services:
- 120 private rooms
- Twenty-four hour skilled nursing care provided by licensed nurses,
certified medical assistants and certified nursing assistants
- Specialized Alzheimer and dementia households
- Short term rehab household
- Hospice
- Certified social service staff
- Full time certified therapeutic recreation staff
Rehabilitation services - Physical Therapy, Occupational Therapy, Speech
Therapy
Twenty-four hour physician services (on call)
Full time Clinical Dietitian
