Primary Programs

Our wide array of services enables our residents to receive the medical care they need, the therapy they require, and the support they and their families deserve. Whether seniors come to us for short-term rehabilitation, or to make their home with us, they can be assured we will provide resident directed care.

Resident Directed Care-  Instead of staff members creating a plan of care for each individual, the residents make their own decisions about things such as waking and sleeping, food preferences, meal times and daily routines.  Our building is designed so that each neighborhood provides a home-like atmosphere. We have the ability to cook, bake and provide activities in the households where individuals live.

Rehabilitations Services-  Lakeland Heath Care Center provides a clinically integrated continuum of post-acute hospital care that helps people regain their lives. We provide a complete range of long term and short term rehabilitation services including physical, occupational, and speech therapy.

Memory Care-  Our memory care household is designated unit for individuals with Alzheimer's' or other dementias. This secured unit offers specialized programs and a supportive environment for those with memory impairment

Restorative Nursing Program- Restorative nursing is a link between the therapy and nursing departments that helps the residents increase and maintain their independance, while retaining their maximum level of functioning. The purpose of the restorative program is to teach, encourage, and enable residents to practice their activities of daily living which helps to build strength and balance.
