Community Health Improvement Process and Plan (CHIPP)
CHIPP is a mechanism to assess the health status of a community and determine the most important health needs to address.  In this assessment, “health” is broadly defined and includes physical health, mental health, environmental health and social health. All areas that contribute to the overall well-being of our residents.

CHIPP is a five (5) year plan that began in Walworth County June, 2011, with the collection of data from a variety of sources, including focus groups, key stakeholder interviews, the Aurora Healthcare Survey, County Health Rankings, and presentations by many Health and Human Services staff. The steering committee identified the current healthcare needs of the community and created an action plan to address them. The CHIPP Steering Committee was made up of several members of the community, with a variety of backgrounds and perspectives on local healthcare concerns. Through the evaluation process the steering committee identified four (4) initiatives to work on over the next four (4) years. They are:

1.     A free medical clinic in Walworth County

2.     Low or free dental care in Walworth County

3.     Promote health and prevent chronic disease across all ages

4.     Develop or coordinate a comprehensive wraparound program for high risk families
