Crisis Case Management

This service is provided to people who are experiencing a mental health crisis or are at risk of experiencing a mental health crisis if services are not provided.

Admission Criteria

·         Person is in crisis or at risk of a crisis situation

·         There is an identified clinical need for the services

·         The need for services will last longer than 2-3 weeks in duration

Included Services

·         Short-term intensive case management (6 months – 1 year)

·         Court commitment/settlement agreement monitoring

·         Therapeutic support

·         Medication management & pharmacological education

·         Risk assessments

·         Crisis planning

·         Service management with providers

·         Assessment of long-term case management needs

Resource coordination (i.e., housing, establishing a primary care doctor, dental care, obtaining benefits, food, clothing, transportation, etc.)
