Comprehensive Community Services

Comprehensive Community Services (CCS) is a voluntary treatment program that provides psychosocial rehabilitation services to children and adults.  The program is designed for consumers that require more than outpatient counseling, but less than the services provided in the Community Support Program (CSP).  Services are provided through the use of a strength based, recovery focused, wrap-around model.  

Admission Criteria

·         Have a qualifying mental health or substance use diagnosis

·         Have a functional impairment that interferes or limits one or more major life activities

·         Found eligible to received CCS services as determined by a State required functional screen

·         Voluntarily seeking services

·         Have Medicaid (Title XIX) insurance

Exclusionary Criteria

·         The following diagnoses are not considered appropriate for admission:

         à Dementia

         à Mental retardation

          à Chronic developmental disability (i.e. Autism Spectrum Disorder)

·         Court ordered

·         Consumer requires medication delivery more than 3 days/week

Included Services

·         Assessment

·         Recovery Planning

·         Individual/group/family counseling

·         Assistance with recovery and life satisfaction goals

·         Medication assistance and education

·         Resource coordination (i.e., housing, establishing a primary care doctor, dental care, obtaining benefits, food, clothing, transportation, etc.)

·         Assistance in learning and accomplishing skills needed to function more independently
