Case Management

This service is for consumers who do not need a Targeted Case Management level of care, but require periodic contact or monitoring.

Admission Criteria

·         Must be 18 or older

·         Have a mental health diagnosis

·         A need to be seen face-to-face, in the community at least  quarterly based on clinical necessity

Included Services

·         Case planning

·         Coordination of services

·         Periodic  symptom management

Resource coordination (i.e., housing, establishing a primary care doctor, dental care, obtaining benefits, food, clothing, transportation, etc.)
